To ensure every behavioral health need is met, we also provide other support services such as Psychiatric Rehabilitation (PRP), Health Homes and Case Management. We also offer consulting and advisory services to other healthcare organizations.

Mobile Treatment Services (MTS) are community-based, intensive, outpatient mental health services designed for individuals who have exhausted traditional forms of outpatient treatment interventions or who have had repeated psychiatric hospitalizations. MTS provides assertive outreach, treatment and support to children, adolescents and adults with mental illness who, according to medical necessity, are unable to utilize traditional outpatient mental health services because of the seriousness of their problems.

HHS Health Homes Program provides hope and support by assisting clients to reach behavioral health goals, addressing somatic health and assisting with overall well being. We coordinate support and other services. Clients are assigned a care provider who helps assess and prioritize physical health needs and then creates a comprehensive care plan.

Please note:

  1. In order to participate in Health Homes, clients must be currently enrolled in either our Psychiatric Rehabilitation Program or Mobile Treatment Services Program
  2. A Care Provider will be assigned to each client.
  3. Services are provided in the client’s home.

HHS child and adolescent Case Management Program provides help and hope to minors with psychiatric illnesses while providing their family with support and access to resources within the community. We utilize a team approach by identifying all supports in the family’s lives and all services are family centered. Families are assigned a care coordinator whom will help assess, prioritize, and advocate for their needs while developing a supportive team of people. Families’ needs are addressed through referral to appropriate services, and through coordination of services with multiple providers and unpaid supporters. HHS will provide a wrap around services to support our client’s growth, independence, and success in their future.

Eligibility Requirements:

  1. Have active medical assistance insurance
  2. Child and Adolescents ages 5-21
  3. Have a persistent mental health diagnosis
  4. At risk for out of home placement, incarceration, inpatient hospitalization, and residential treatment

Our Adult Targeted Case Management (TCM) program is designed to provide comprehensive support to adults with mental health needs, ensuring they have access to the services and resources necessary for their well-being. This program is aimed at individuals who require assistance in navigating complex systems of care, including health services, social support, and community resources.

Program Features:

  1. Personalized Care Coordination: Each participant in the TCM program is assigned a dedicated case manager who works closely with them to develop a personalized care plan. This plan addresses the unique needs of the individual, focusing on achieving specific health and wellness goals.
  2. Access to Resources: Our case managers connect participants with a variety of services, including mental health treatment, medical care, housing assistance, employment support, and more. We ensure that all necessary resources are accessible to help individuals lead stable and productive lives.
  3. Ongoing Support: The TCM program provides continuous support, with case managers regularly monitoring progress and adjusting care plans as needed. We emphasize building long-term relationships to ensure sustained success and improvement in quality of life.
  4. Advocacy and Empowerment: We advocate for the rights and needs of our participants, ensuring they receive appropriate services and support. Our goal is to empower individuals to take an active role in their care and make informed decisions about their future.
  5. Collaborative Approach: The TCM program involves collaboration with various service providers, community organizations, and family members to create a strong support network for each participant. This holistic approach ensures that all aspects of an individual’s life are considered in the care planning process.

Please note that our Adult TCM program exclusively serves clients residing within Baltimore City. We are dedicated to supporting the local community and ensuring that Baltimore City residents have access to the services they need.

Eligibility:

The Adult TCM program is available to adults who have a diagnosed mental health condition and are in need of assistance with coordinating care and accessing services. Participants must meet specific eligibility criteria, which can be discussed with one of our intake coordinators.

Program/Treatment Model

Hope Health System’s child and adolescent PRP program is goal directed, outcome focused, and time-limited interventions designed to reduce maladaptive behaviors and to restore and strengthen specific age appropriate skills so that the youth can function to their highest potential up to and including independence. PRP treatment is seen as a planned and integrated adjunct to outpatient mental health treatment, and the total plan of care or the youth. PRP includes a combination of psycho-education, emotional and behavioral awareness competency, and social skills training in groups or individually, at the agency site, or in the youth’s home or in the community.
The focus of the Psychiatric Rehabilitation Program treatment model will be directed toward the reduction of emotional or behavioral problems, and the restoration of age-appropriate skills, including:

  1. Facilitating the enhancement of an individual’s independent living and social skills, including the individual’s ability to make decisions about his or her life, while creating opportunities for choice regarding home, school or work, or community
  2. Promoting community resources to integrate the individual into the community.

Rehabilitative activities (in individual sessions, psycho-educational groups, and family groups, as appropriate) that will be incorporated in the PRP include:

  1. Self-care skills (i.e., hygiene, grooming, dietary planning, food preparation, age-appropriate involvement with self-administration of medication)
  2. Social skills (i.e., conflict resolution training, improved peer interactions, anger management, improved interactions with authority figures, assertiveness training, assessing risks in social environments)
  3. Independent living skills (i.e., maintaining personal living space, community awareness, mobility skills, monetary management, housekeeping, the importance of punctuality, task analyses, completion of chores, staying safe while in the community, education on age-appropriate meal preparation, doing laundry)
  4. Leisure and recreational activities (i.e., development of healthy leisure activities, use of leisure to develop teamwork and peer support behaviors, promotion of productive and rewarding use of time spent alone)
  5. Schedule planning (i.e., daily schedule planning, time management, development of daily routine, development of routine before going to bed at night to prepare for the next day)
  6. Cultural interests (i.e., exploration of activities that support the youth’s cultural interests by self-expression through artistic modalities)
  7. Age-appropriate techniques and resources to provide understanding of the role and effects of medication and symptom management; and
  8. Age-appropriate techniques and resources to understand basic healthy living, nutrition, exercise, sexuality, and health education/risk reduction.

The Correctional and Aftercare Mental Health Services program is offered at the Baltimore City Juvenile Justice Center (BCJJC (DJS)). This program provides mental health services to youth in the juvenile center.

Centurion Health understands that mental health is key to improving the whole health of the individual. That’s why HHS and Centurion are committed to providing behavioral healthcare services that don’t end when individuals re-enter their community. As a leading correctional healthcare provider in Maryland and across the U.S., Centurion has the perspective and experience to meet the needs of incarcerated populations. We partner with organizations like Centurion to help individuals integrate into society while maintaining access to important mental health services.

Partnering with Centurion Managed Care, HHS provides community re-entry programs for released inmates and serves as a direct link to community resources. HHS’ goal is to provide more integrative services in addition to medication management, to have a continuum of services available to released inmates, to provide reciprocal training and education opportunities and to ensure continuity of care for transferring inmates on the mental health caseload.

Hope Health Systems, inc. has been providing mental health and substance abuse services to detained youth at the Baltimore City Juvenile Justice Center, also known as the Department of Juvenile Services (DJS), for over a decade. Our clinical team has a proven track record of providing clear results. At the Baltimore City Juvenile Justice Center, HHS was able to have all of the existing Civil Rights of Institutionalized Persons ACT (CRIPA) deficiencies removed. Other achievements at the Juvenile Justice Center include:

  1. Implemented mental health assessments.
  2. Developed suicide protocols and suicide prevention training.
  3. Protocols for treatment planning.
  4. Improved record keeping protocols.
  5. Implemented standards for providing access to adequate mental health treatment within a reasonable time frame.

Juvenile Justice

Research has established that the vast majority of youth in the juvenile justice system (approximately 70%), suffer from mental disorders, with many experiencing disorders so severe that their ability to function is significantly impaired. A youth’s contact with the juvenile justice system is often the first and only chance to get help, or it may be the last resort when one system after another has failed. Juvenile justice administrators and communities are struggling to make progress in creating systems that support the specialized needs of these youth.

Services offered at Juvenile Centers

HHS provides a range of mental health and substance abuse treatment services to youth involved in the juvenile justice system. Currently HHS provides an interdisciplinary program of mental health and substance abuse treatment at the Baltimore City Juvenile Justice Center, in partnership with local and state agencies and other community-based organizations.

The comprehensive juvenile justice services that HHS provides include: psychiatric evaluation, psychological testing, medication management, counseling (individual, group and family), suicide assessments, crisis interventions and referral services, aggressive reduction therapy and training for direct care staff on de-escalation skills and peer relations. HHS Juvenile Justice services are provided based on an integrated treatment model with care providers who have extensive training in treating youth who have mental health and/or substance abuse disorders.
